DATA PRIVACY CONSENT

In compliance with the Data Privacy Act (DPA) of 2012, and its Implementing Rules and Regulations, we agree and authorize NAFECOOP to:

a. Use our Personal Information or data to process any transaction related to the availment of NAFECOOP's products and services including the insurance services and administer the benefits as stated in our policies and other existing service agreements.

b. Retain our information in NAFECOOP’s Membership Information Database in all its branches shared with other entities like CLIMBS Life and General Insurance Cooperative and its subsidiaries.

c. Share our information to affiliates and necessary third parties for any legitimate business purpose. With the assurance that security contingencies are employed to safeguard our information.

d. Retain our personal data for the establishment, exercise, or defense of legal claim, for legitimate purposes and for the fulfilment thereof. Accordingly, for the purposes allowed by law.

e. Use all our personal information or data, transactions, membership information and relevant documents for the protection and enforcement of rights and interest of NAFECOOP in the preparation of demands, conciliation, mediation, and court proceedings or regulation.

f. Inform us of future customer campaigns and base its offer using the personal information we have shared or will share with the company.

The provisions of this agreement, the Articles of Cooperation, By-Laws, and Data Privacy Consent have been explained to us and we agree to abide by all of them.

Per Article III of NAFECOOP’s Articles of Cooperation:

The goals of this federation are to help improve the quality of services of its affiliates and in furtherance thereto shall aim to do the following pursuant to the functions of federation under article 24 of RA 9520:

1. To carry on any cooperative enterprise authorized under Article 6 of RA 9520 that compliments, augment, supplements but does not conflict, compete with nor supplant the business or economic activities of its affiliates;

2. To carry on, encourage and assist educational and advisory work relating its affiliate cooperatives;

3. To render services designed to encourage simplicity, efficiency, and economy in the conduct of the business of its affiliates and to facilitate the implementation of their bookkeeping, accounting, and other systems and procedures;

4. To print, publish, and circulate any newspaper or other publication in the interest of its affiliates and enterprises;

5. To coordinate and facilitate the activities of its affiliate cooperatives;

6. To enter into joint ventures with national or international cooperatives in the manufacture and sale of products and/or services in the Philippines and abroad; and

7. To perform such other functions as may be necessary to attain its objectives.

One of the primary goals of the NAFECOOP, in collaboration with the CLIMBS Institute of Management (CIM) is to provide continuous learning, professional cooperative management, and higher level of knowledge focusing on soft-skills development.

MEMBERSHIP SUBSCRIPTION AGREEMENT

The undersigned Chairperson/GM/CEO hereby agrees to subscribe to the capital share as approved by the Board of Directors (BOD) of NAFECOOP set forth in this Subscription Agreement.

By execution of this Subscription Agreement, the undersigned Chairperson/GM/CEO hereby acknowledges the understanding and agreement to comply with the obligations and duties expected of their Cooperative; which are as follows:

1. To comply with the provisions of the Articles of Cooperation, By-Laws, the policies set by the Board, the General Assembly, as well as the directives of duly constituted authorities.

2. To comply with the directives of the duly constituted authorities as well as decisions of the Board of Directors regarding the operating policies of NAFECOOP.

3. To attend meetings, conferences, seminars, and/or online events as required by NAFECOOP or its BOD.

4. To pay the one-time Membership Fee of five-hundred pesos (₱500).

5. To participate in the Capital Build-up program of NAFECOOP by:

a. Subscribing for at least ten (10) shares valued not more than 10% of the total paid up capital at ten-thousand pesos (₱10,000) or as determined by the BOD per share, paying for the subscribed shares in lump sum from the date of membership.

b. Contributing five-thousand pesos (₱5,000) as Annual Dues which is automatically deducted from the CETF. Section 5.b has been waived per NAFECOOP BOD decision

c. Investing in the Cooperative Education and Training Fund (CETF) which will be utilized thusly:

i. 70% Consumable on training including expenses, per diem, and incident expenses.

ii. 20% Redeemable investment training fund without interest (a.k.a “Knowledge Investment”).

iii. 10% Advocacy fund. By affixing our signatures below, We are fully aware and have come to understand the provisions of this Agreement, the Articles of Cooperation, and By-laws. Hence, We agree to abide by the abovementioned conditions as well as the imposition of sanctions against us in case of commission of acts not in accordance or against the aforementioned provisions.
